Our top priority as an oral healthcare team is to provide our patients with the highest quality of care specific to each patient’s needs. We will provide a written treatment plan, with your costs clearly outlined when any treatment is recommended. Lakeville Dental Membership Plan
For those patients who are looking for an alternative to dental insurance, deductibles, frequency limits, or out-of-pocket maximums, Lakeville Dental offers dental membership plans. Our membership plans cover all your preventative care, routine x-rays, and savings on other treatment procedures. Plan benefits start immediately, and membership runs for 12 months from the date you join. There are no deductibles, waiting periods, annual maximums, denial of claims, or pre-approvals. Our membership plan also includes 15% off all procedures completed at our office and products sold in our office. This includes cosmetic procedures, Invisalign, in-office and at-home whitening.
Lakeville Dental Membership plans are NOT insurance and cannot be combined with any other insurance benefits, offers, or discounts.
Dental Insurance Plans
Lakeville Dental is an unrestricted provider and is not contracted with any dental insurance provider. We are an insurance-friendly office and work with many different insurance companies and plans. As a courtesy to all our patients who have PPO dental benefits, we will file your claims electronically and work with your dental benefits provider to help maximize your benefits. If you have a change in dental benefits, please call our office at least 48 hours before your appointment so we can update your records and ensure you receive the correct dental benefits. Our office is always available to help review your benefits plan with you and assist with any questions regarding your benefits plan coverage.
Payment Information
We accept most major credit cards in the office or online, including American Express, Discover, Mastercard, and Visa. We also accept mobile wallet apps such as Apple Pay®, Google Pay™, and Samsung Pay®.
CareCredit®

Cancellations
Kindly provide at least 48 hours notice for any appointment that needs to be rescheduled. Appointments cancelled in less than 24 hours may be subject to a cancellation charge.
